If you are interested in starting a club or organization please fill out the following paperwork and turn it in to the office or ASB President:
You will need to be logged in to your student PVUSD account to access most files.
REQUIRED: Club/Athletics Charter (make a COPY and edit as you feel fit)
Example Club/Athletics Charter (you can also google 'Club Charter Examples' to view more)
REQUIRED: Club/Athletics Signature Card (this tells ASB and the principal who is able to request your account balance, request for fundraiser activity, request for money to purchase items for the club and make deposits to ASB)
Other Club/Organization Forms:
Club/Athletics Minutes (You must submit this to ASB when voting for expenditures or voting on any matter)
Fundraising Activity Request (Submit this 2 weeks prior to the proposed date)
Cash Receipt Log (Use this when collecting money for fundraisers and submit it along with your deposit to ASB or the principal)
